Participation Requirements

• Must have physical dated after 4/15/25 before starting any practice (Grades 4th-12th) Turn into main office

• Must have activity fees paid before first contest (all ages) Pay at main office

• Must have concussion baseline assessment completed before first contest (Grades 7th-12th) Contact Mr. Larson if didn't complete spring '24

• Must view below required presentation and have consent signed before any contest (all ages)
